The contents of FDA registration information

- the name, address, and telephone number of the plant and the Corporation (if applicable);
- name, address, and telephone number of the owner, operator or agent;
- all trade names used by the plant;

- to prove that the submitted information is true and accurate, the author (if not the owner, operator or agent) is authorized to submit a statement of information.
- the name, address and telephone number of the American agent must be provided by foreign factories;
- foreign factories also need to provide emergency contact phone calls to their U.S. agents, unless the plant is specified by other emergency contacts.
